How GLP-1 microdosing is actually done, if you’re considering it
The short answer
Before anything else: read our overview of the evidence. There is no clinical trial data showing microdosing works, and the practice is not endorsed by AACE. This page describes how it is done in practice for people who are going to look this up either way — it is not a recommendation, and the single biggest safety factor is whether a prescriber who knows your history is actually involved.
Supervised versus self-directed: the distinction that matters most
“Microdosing” covers two very different things that get talked about as one:
- A prescriber intentionally keeping you at a low, approved-increment dose — for example, staying at an early titration step longer than the standard schedule because that is where you tolerate the medication, under their monitoring. This uses the same approved-strength pens as standard treatment; it is a dosing decision, not a different product.
- Buying a compounded product marketed specifically as a “microdose,” or measuring a fraction of a dose yourself — this is the version with the documented safety problems: inconsistent compounded dosing, and real measurement error from self-splitting a dose designed to be given as a single unit.
The first is a legitimate clinical conversation to have with your prescriber. The second is the version physicians are specifically warning about.
Why self-measuring a fraction of a dose is riskier than it sounds
Standard GLP-1 injector pens are designed to deliver a specific, fixed dose reliably; they are not designed as a "draw out a fraction" device, and doing so introduces real measurement error. Compounded products sold specifically as microdoses add a second layer of uncertainty, since the FDA does not review compounded formulations for manufacturing consistency the way it does an approved product — meaning the actual dose in the vial may not precisely match the label.
If you are going to raise this with a prescriber, ask
- “Is there a clinical reason to keep me at a lower dose longer, rather than following the standard titration schedule?”
- “If I’m struggling with side effects, is a supervised dose reduction the right move instead?”
- “Are you able to prescribe and monitor an off-label low-dose approach, or is that outside what you offer?”
- “What would you actually want to monitor if we did this?”
What we would not recommend
- Buying a product marketed as a “GLP-1 microdose” from a seller who has not evaluated your medical history.
- Drawing a fraction of a standard-strength pen yourself without a clinician specifically instructing you how and confirming the resulting dose.
- Treating microdosing as a lower-risk or “gentler” alternative to standard treatment — the evidence gap, not the dose size, is the actual issue.
